Millicent's Quest follows a long optional storyline from a Scarlet Rot-ravaged church in Caelid to the roots of the Haligtree. Its major reward is determined by whether you aid Millicent or invade her, and the assist route can eventually produce an item that affects the Frenzied Flame ending.

The quest begins in Caelid with Gowry, an elderly sage found at Gowry's Shack, and Millicent, a young woman suffering from Scarlet Rot at the Church of the Plague. Gowry asks you to recover an Unalloyed Gold Needle from Commander O'Neil in the Swamp of Aeonia. After the needle is repaired and given to Millicent, she can move through several regions as the player advances, making this storyline easy to interrupt or leave unfinished if its characters are not revisited.

Millicent's route is not a single dungeon or mission area. Her appearances progress from Caelid to the Altus Plateau, then through the Mountaintops of the Giants and into Miquella's Haligtree. The important progression checks are speaking to her after each relocation, defeating the local obstacle at Windmill Village, and reaching the Prayer Room before entering the drainage area beneath the Haligtree.

The decisive encounter occurs after defeating the Ulcerated Tree Spirit near the Drainage Channel. Two summon signs appear in the area: one lets you assist Millicent against her sisters, while the other lets you challenge Millicent. These options are mutually exclusive for that playthrough at this stage. The assist route continues after the Haligtree battle.    Begin the needle portion of Millicent's Quest

   Speak with Gowry at Gowry's Shack

   Travel to Gowry's Shack in eastern Caelid and exhaust Gowry's dialogue. He explains that an Unalloyed Gold Needle is needed to help Millicent, and identifies Commander O'Neil in the Swamp of Aeonia as the person carrying it.

   Location: Gowry's Shack, Caelid

   TipUse the shack as the starting reference for the nearby Swamp of Aeonia, but do not assume the quest advances simply by finding Millicent before obtaining the needle.

2. 02

   Recover the Unalloyed Gold Needle

   Defeat Commander O'Neil

   Enter the Swamp of Aeonia and defeat Commander O'Neil, taking care around the swamp's Scarlet Rot buildup and the boss's summoned soldiers. Loot the Unalloyed Gold Needle after the fight; it is the quest item Gowry needs, not a general-purpose weapon.

   TipKeep the fight away from the deepest rot whenever possible and remove pressure from the summoned soldiers before committing to long attacks on Commander O'Neil.

   Have Gowry repair the Unalloyed Gold Needle

   Return the needle to Gowry

   Bring the Unalloyed Gold Needle back to Gowry at Gowry's Shack and give it to him. Leave the area and return, or rest and reload as needed, so Gowry has time to work on the damaged needle before speaking to him again.

   Treat Millicent's Scarlet Rot

   Give the repaired needle to Millicent

   Go to the Church of the Plague and speak with Millicent, who is positioned inside the church in a weakened state. Give her the repaired Unalloyed Gold Needle, then rest or reload and speak with her again to complete this stage.

   Defeat the Godskin Apostle at Windmill Village

   Clear the route through Windmill Village and defeat the Godskin Apostle at the village's upper area. Once the boss is defeated, rest at the nearby Site of Grace and speak with Millicent at the village; the post-boss rest is important for updating the scene.

   TipThe Godskin Apostle's attacks punish close-range greed, so use the village space to create distance after its long attack strings before re-engaging.

    Unlock the final choice

    Defeat the Ulcerated Tree Spirit

    From the Prayer Room route, reach the lower area near the Drainage Channel and defeat the Ulcerated Tree Spirit. After the boss is dead, reload the area if necessary so the gold and red summon signs appear near the pool where the fight occurred.

    TipThe arena gives the Ulcerated Tree Spirit room to move, but its sudden lunges can carry it across the pool; watch its body rather than attacking through the visual effects.

    Take the assist route

    Assist Millicent against her sisters

    Interact with the gold summon sign reading that you will assist Millicent. Enter her world and help her defeat the invading sisters, then return to your world and reload the area before speaking with Millicent near the summon pool.

When she seems to have vanished, first check whether the previous conversation was exhausted, whether the relevant boss has been defeated, and whether the area has been reloaded. This is especially important after giving the needle to Gowry, after defeating the Godskin Apostle, and after the Haligtree summon encounter.

The Unalloyed Gold Needle changes purpose during the quest. Initially, it is the damaged item recovered from Commander O'Neil and repaired by Gowry for Millicent's treatment. After the assist branch, the needle is recovered again and can be taken to the flower in Malenia's arena, where it becomes Miquella's Needle rather than remaining a simple quest object.

The Haligtree confrontation is a mutually exclusive reward check. The gold summon sign is the cooperative option and leads to the Rotten Winged Sword Insignia, while the red summon sign is the hostile option and leads to Millicent's Prosthesis.
